Please read this before any paid work. Here is the order of steps:
This Agreement is between you (the “Participant”) and John Blanche, LLC d/b/a Reset Coaching (“we,” “us,” or “Reset Coaching”). It is a coaching agreement.
It is practical one-on-one coaching for adults who want a steadier mind and habits, healthier relationships, and room to pursue what matters. We work on organization, focus, accountability, following through on your goals, and building how you see yourself (self-esteem). How you see yourself is a core focus of this work, and we say so openly.
It is not therapy, medical care, crisis care, or any other licensed health service. We do not diagnose or treat mental-health conditions, and coaching does not replace care from a licensed professional.
If you need that kind of care, keep it or start it. You are welcome to work with a licensed professional alongside coaching.
Optional physical activities. Coaching may include gentle, optional activities like slow breathing, short walks, or building regular exercise into your plan. You can always skip them. You’re responsible for choosing what’s safe for your body and for checking with your doctor if you have a health condition. Stop anything that causes pain, dizziness, or distress. Reset Coaching doesn’t give medical or fitness advice.
If you are in crisis, thinking about harming yourself or others, or need emergency help:
Do not wait for a coaching session.
If safety comes up during coaching, John will pause the coaching agenda, ask you directly about your safety, and help you connect with 988, emergency services, or a licensed professional. If John believes someone is at serious risk, he may contact emergency services or your emergency contact. Details are in the Privacy & Records Notice.
As part of your intake paperwork (after your free consult and before kickoff), you’ll take a short self-reflection questionnaire about how you see yourself, and again around week 8. It’s part of how the program works, so we tell you about it before you sign up. You can choose “Prefer not to answer” on any question. Each time, you read and sign a short consent first.
Services: a free 25-minute consult, a 60-minute kickoff, 50-minute ongoing sessions, and 4-week and 8-week reset packages.
Fees. After your free consult, John sends you a dated written Fee Quote (email or PDF). It lists the service or package, the price, what’s included, the single-session rates (kickoff and ongoing session) used for refunds, and the amount of any late-cancel or no-show fee. The Fee Quote is part of this Agreement. You accept it by checking the terms box and paying through the payment link (or by replying “I accept”). Nothing is owed until you accept it. Fees may change for future work; a paid package keeps the price you paid unless we both agree otherwise in writing.
Payment: paid services are due before the session through a secure payment link we send you, only after your signed intake and Agreement are in. Unpaid balances may pause scheduling.
You agree to give accurate intake information; tell John if your safety situation changes in a way that matters for coaching; do the between-session work you agree to, as you are able; keep any care you already get from a licensed provider; and take scheduling and payment commitments seriously.
John agrees to stay within coaching (not therapy); keep your information private except as described in the Privacy & Records Notice; never talk about you as gossip or entertainment; refer you to a licensed professional or pause coaching when crisis needs or other care come first; and keep careful records for continuity and accountability.
Coaching supports change, but it does not guarantee any particular outcome, including job, relationship, or mood results. You remain responsible for your own decisions and actions.
Your written Fee Quote lists what your package includes (number and length of sessions), the price, and the single-session rates used below (the kickoff rate and the ongoing-session rate).
Either of us may end coaching at any time. Whoever ends coaching, the cancel rules still apply and any refund follows Section 11. John may end or pause coaching if it is not a good fit, if safety needs call for a referral, if policies are repeatedly broken, or if continuing would be unethical. When ending for safety reasons, John will try to give you a clear referral path. If John pauses coaching to refer you to licensed care, coaching can resume once you confirm in writing (email is fine) that you’re connected with a licensed professional and want to continue. We’ll keep coaching to your day-to-day goals, and we encourage you to let your therapist know. John will talk with your therapist only if you ask him to in writing.
The Privacy & Records Notice is part of this Agreement. It explains what we collect, how we use it, how long we keep records, and the limits of confidentiality.
Confidential, but not privileged. What you share in coaching is kept confidential under this Agreement. But coaching conversations are not legally privileged the way conversations with a therapist or a lawyer can be. A court order or subpoena could require John to share them.
Worksheets, scripts, and other materials John provides remain the property of John Blanche, LLC. You may use them for your own coaching work, but please don’t republish or sell them.
To the fullest extent Pennsylvania law allows, John Blanche, LLC’s liability related to coaching is limited to the fees you paid for the specific services involved in the 12 months before a claim. This limit does not apply to harm caused by gross negligence, recklessness, or intentional misconduct. This does not limit any liability the law does not allow us to limit.
Each of us is responsible for our own choices. You agree to cover John Blanche, LLC’s reasonable costs, including reasonable legal fees, if someone else makes a claim against it arising from your breach of this Agreement or your own unlawful conduct, unless the claim is caused by John Blanche, LLC’s own carelessness or misconduct. In the same way, John Blanche, LLC agrees to cover your reasonable costs if someone else makes a claim against you because of its own carelessness or misconduct in providing coaching. Section 15 applies to John Blanche, LLC’s obligations under this section. This section does not limit any rights the law does not allow to be limited.
Small claims and urgent matters. Either of us may skip steps 1 and 2 for a claim that fits within the small-claims limit of Philadelphia Municipal Court (currently $12,000), or to ask a court for urgent relief.
This Agreement is governed by the laws of the Commonwealth of Pennsylvania, without regard to conflict-of-law rules.
You agree that we may do business electronically. To sign and receive records electronically, you need an email account and a device that can open web pages and PDFs. Tell John if your email address changes. You can ask for any record on paper at no charge. Checking a box and typing your full name, or signing electronically, counts as your signature. Electronic copies of this Agreement, your intake, and any consent you sign are valid records, just like paper. John keeps a dated copy of each signed intake and Agreement (for example, the submission email saved as a PDF). You can ask for a paper or PDF copy at any time at no charge. You can also withdraw your consent to electronic records by emailing John; that does not undo anything already signed.
This Agreement, the Privacy & Records Notice, your completed intake (including your checked acknowledgments and typed signature), and any self-reflection questionnaire consent you sign are the full agreement for coaching services. Verbal side deals do not replace these written terms. If one part is found invalid, the rest stays in effect.
You accept this Agreement by checking the acknowledgment on the intake form and typing your full name as your signature, or by signing a copy John sends you. You do this before you pay for anything. You can ask for a PDF copy at any time by emailing johnblanchejr@gmail.com.
